This is the shortest collection page we have, on purpose. When we rated every mattress we sell on the same 1-10 scale, exactly four measured a genuine 9: the Sealy Posturepedic Elite Brenham Ultra Firm, the Sealy Posturepedic Plus Plainfield 13-inch Ultra Firm, the Stearns & Foster Estate 14.5-inch Ultra Firm, and the Stearns & Foster Lux Estate Ultra Firm. Everything else wearing an Extra Firm or Ultra Firm label in our building measures an 8, and you will find those beds listed honestly on our firm mattress page.
The folklore problem
Extra firm carries decades of received wisdom: bad back, get the hardest bed in the store. That advice dates to an era when the alternative was a collapsing innerspring, and the research has moved on. Rock-hard is not automatically therapeutic. For many sleepers, a surface with zero give pushes the shoulders and hips up instead of letting them settle, and the lower back bridges unsupported. The people who genuinely thrive on a 9 tend to be stomach sleepers, sleepers with higher body weight who compress an 8 into feeling like a 6, and people who simply grew up on hard beds and sleep best on what their body knows. If that is you, these four beds are built for it.
What a true 9 is actually like
An 8 still greets you with a comfort layer. A 9 is nearly all support system: dense coils, minimal padding, a surface your body lies on top of rather than into. Before you commit to the hardest bed we sell
Two honest checks. First, if your current bed feels too soft, make sure the mattress is the problem: a worn foundation makes a decent bed feel like a hammock, and a bunkie board under your current mattress is a far cheaper experiment than a new 9. Second, spend real time on a true 8 the same day you try these. Lie on both for ten minutes each, in your actual sleep position. Your shoulders will cast the deciding vote, and they are more trustworthy than folklore.

Who should sleep on an extra firm mattress?
Three groups do best on a true 9: dedicated stomach sleepers who need hips held level, heavier sleepers who compress a standard firm bed past its intended feel, and people whose bodies are simply adapted to very hard sleep surfaces. For most everyone else, a true 8 delivers the support without the pressure points.
Is an extra firm mattress good for your back?
Only if it fits your build and position. The old advice that a bad back needs the hardest bed available has not held up; surfaces associated with better outcomes for most back-pain sufferers are medium-firm to firm, not maximum-hard. A 9 can help specific sleepers, especially stomach sleepers, but it is a fit decision, not a prescription.
What is the difference between extra firm and firm?
A firm bed, an 8 on our scale, still has a comfort layer you can feel working. An extra firm 9 is nearly all support layer with minimal padding. The difference on your shoulder after ten minutes is bigger than the one-point gap suggests, which is why we suggest trying both the same day.
Why do you only have four extra firm mattresses?
Because we rate by actual feel, not by label. Several beds we carry are sold as Extra Firm or Ultra Firm but measure an 8 on the same scale we apply to everything, so they live on our firm page. The four beds here are the ones that genuinely measure a 9. A longer list would be a less honest one.
Do I need a box spring under an extra firm mattress?
You need the correct rigid support, which is usually a foundation or platform rather than an old-style springy box. A true 9 on a flexing base loses the exact quality you bought it for. If your frame is older, budget for a proper foundation; it is the difference between the bed you tried and the bed you get.
Will an extra firm mattress soften up?
Marginally. With so little comfort material, a 9 has less to break in than any other feel, which is part of the appeal: the bed you try is very close to the bed you will have in year five. Expect the surface fibers to settle slightly in the first month and little change after.
